A receipt of payment of electricity fees to the work center and in the name of the person concerned and/or a receipt of payment of landline phone bills in the name of the person concerned and at the work center.
Or an occupancy statement from the municipality that proves the continuity of the establishment's business.
In the event that a personal affiliation card with a photo is requested, a photograph must be submitted to the authorized signatory on behalf of the institution or company, certified by the Mukhtar.
Show official invoices (purchasing and selling) proving the commercial activity (no less than five invoices).
Or an industrial certificate or any valid license to practice a profession that proves the activity.
A copy of the lease contract certified by the municipality in the name of the institution or company with the latest municipal receipt, and in the absence of a municipality, the approval of the local mayor is required.
Or a copy of the title deed in the name of the affiliate (natural person or legal person).
Or a copy of a title deed in the name of one of the partners, attached to a letter of grace submitted by the partner, whereby he puts the property at the disposal of the company and approves it as a work center for it, for the duration of his contribution to the company, authenticated by a notary public.
A recent real estate statement if the affiliate is an owner (not more than three months old).
A copy of a free-management investment contract registered in the commercial registry and attached to a copy of the announcement in the Official Gazette.
A letter in which the institution or company explains the type of new activity and documents indicating this new activity, provided that this activity is among the activities originally declared in the commercial registry and is identical to the purpose of using the leased property.
